In addition, an emergency contact number is required in case of an accident or injury during class.

Please enter your name, give or deny permission for the club to use your photo in promotional material, and at least an emergency contact number. Other fields are optional, but we would appreciate the additional information. All personal information is encrypted in our database, and only our chief instructor can access it when necessary.
